If you are accepting, please be sure to provide your date of birth in the space provided on your letter. The purpose of requesting your birth date is to assist us in establishing your employment record in our information management system.

Please complete the Inventions & Secrecy Agreement below, filling out any patents that may be applied. Please wait to sign the document until it can be witnessed. This must be signed in front of a Vetmedica representative:

You will be sent a separate email with the subject line: Electronic Form I-9 Section 1 Invitation. This email will contain instructions to begin the I-9 verification process.
Section 1 must be completed prior to your first day. This is a Federal requirement.
Please note that you can use the provided log-on only once . Therefore, you will need to have the following information available when you are ready to complete Section 1: Social Security Number and if you are not a US Citizen you will need to have your Admission or Alien Number, with expiration date, available in order to complete Section 1.
Please refer to the I-9 Form information link to see which documents you can chose to present on your first day. Please bring the original documents and one copy with you on your first day.

Medical, Dental and Vision benefits are effective the first day of the month that is the same as or next following the employee's date of hire.
If you are hired on May 15th, you are eligible for these beneifts on June 1st.
If you are hired on May 1st, you are eligible for these benefits immediately.
